Business Rules
Upon approval DIA staff are entitled to 1 eye exam per year (Corporate Benefits Eye Exam $30 or CL Assessment & Half price EE $60) and every 2 years a $350 contribution towards corrective glasses or contact lenses. Any amount exceeding $350 is at the cost of the staff member (other than the mentioned services). If the staff member is an AA member the 1 FREE eye exam every 2 years can be used in conjunction.
